Most people you refer need more than one thing, and the things change. Finding a place to live. Getting through a day. Getting ready to work. Getting a job. Keeping it. We do all of that, so a change in what someone needs isn't a new referral to a new agency.

  1. Somewhere to liveHelp working out housing options and getting through the applications and waitlists. Chore services to keep a place livable once someone's in it.
  2. Getting through the dayIn-Home Support and Supported Living for daily living. Day Habilitation for structure, skills and being out in the community rather than at home alone.
  3. Getting ready to workAssessment and job readiness — figuring out what someone can sustain before anyone puts them in front of an employer. For students still in school, Pre-ETS.
  4. Getting the jobJob search and placement, worked as a match rather than a placement count.
  5. Staying in itOn-the-job supports that fade as they're earned, and daily supports that continue underneath for as long as they're needed.

People move around this list in both directions, and often need two parts of it at once. That's the point — the plan changes without the provider changing.

There's more than one way to pay for services, including private pay when someone is waiting on other funding. You don't need to know which before you call — sorting that out is part of the first conversation.
